in previous lecture we have learned
- Pneumococcal pneumonia
- staph pneumonia
- their c/f,investigation,risk factors and treatment
in detail.
So now we will continue our discussion with
Bacillus anthracis pneumonia(biological weapons)
this pneumonia is also known as wool-sorters disease
Agent:
Gram positive rods (spore forming bacteria)
source:
spores of cattle wool, so it is also categorised as Zoonotic disease.Because this disease is caused by inhalation of spores
Risk factors:
- cattle handler
- form workers
- abattoir
Clinical features:
- high grade fever
- productive cough with sputum production
- dyspnea
- chest pain(pleuritic chest pain)
Invstigation:
raised WBC’s
Investigation of choice:
sputum culture
Alternate investigation:
blood culture
Chest X-Ray:
broncho-pneumonia
Treatment:
inj benzyl penicillin
Alternate treatment:
ciprofloxacin
Pseudomonas pneumonia
Agent:
gram negative rod (enteric)
Mode:
- Hospital acquired pneumonia( HAP) – Leading cause
- Community acquired pneumonia(CAP) – 1-2% cases
Special risk factors for CAP:
- pre-existing lung disease
- COPD
- ILD
- Bronchiectasis
Clinical feature:
- high grade fever
- productive cough(greenish sputum)
- dyspnea
- chest pain(pleuritic chest pain)
General physical examination:
bronchial breath and coarse crepitation
investigation/investigation of choice/alternate investigation/chest X-ray:
all these are as same as Bacillus anthracis pneumonia
Treatment:
inj ceftriaxone OR Levofloxacin/moxifloxacin
if complication(septic shock/DIC)
